  • Pass a strenuous background check 

  • Be at least 21 years of age.

  • Be financially stable.

  • Valid driver’s license, reliable transportation and insurance.

  • Willing to obtain liability insurance.

  • Ability to read, write and speak English. [English does not have to be the primary language.] 

  • Be physically and emotionally healthy and able to perform their duties.

  • Provide a safe, stable, and secure environment for the adult with disabilities.

  • Role model healthy family living. 

  • Complete all required training.

  • Own or rent your own home. 

  • Willingness to complete a home study.

  • Have an extra bedroom for the person(s) they are willing to support. 

  • Ability to implement the person-centered support plan.

  • Be willing to have the support coordinator and program coordinator complete routine home visits.

  • Provide updates on the person supported on a regular basis. 

  • Complete daily documentation of supports and services.

  • Involve the person(s) in their home as a family member and in their local community. 

  • Assist in coordination of medical needs and appointments.
