QUALIFICATIONS OF FAMILY MODEL PROVIDERS

Pass a strenuous background check

Be at least 21 years of age.

Be financially stable.

Valid driver’s license, reliable transportation and insurance.

Willing to obtain liability insurance.

Ability to read, write and speak English. [English does not have to be the primary language.]

Be physically and emotionally healthy and able to perform their duties.

Provide a safe, stable, and secure environment for the adult with disabilities.
-
Role model healthy family living.
-
Complete all required training.
-
Own or rent your own home.

Willingness to complete a home study.
-
Have an extra bedroom for the person(s) they are willing to support.

Ability to implement the person-centered support plan.
-
Be willing to have the support coordinator and program coordinator complete routine home visits.
-
Provide updates on the person supported on a regular basis.
-
Complete daily documentation of supports and services.
-
Involve the person(s) in their home as a family member and in their local community.
-
Assist in coordination of medical needs and appointments.
